To access the external jugular vein, place the patient in Trendelenburg position with the head tilted slightly contralaterally. Do a preliminary inspection nonsterile to identify a suitable vein: Apply a tourniquet, have the patient make a fist, and palpate using your index finger to locate a large-diameter vein that is nonmobile and has good turgor.

To help distend and locate veins, tap a potential site with your fingertips. It may help to allow the arm to hang down, increasing venous pressure. Use a vein-finder device if a suitable vein is not readily seen or palpated. Apply anesthetic if it is being used and allow adequate time for it to take effect eg, 1 to 2 minutes for gas injector, 30 minutes for topical.

Cleanse the skin site with antiseptic solution, beginning at the needle-insertion site and making several outwardly expanding circles.

Wait for the antiseptic solution to dry completely. If applying povidone-iodine , wipe it off with alcohol and allow the alcohol to dry. If blood is being obtained for blood cultures , vigorously cleanse the site with alcohol for 30 seconds, allow the alcohol to dry, and then swab in outwardly expanding, overlapping circles using chlorhexidine or povidone-iodine.

Wait for the antiseptic effect to occur 1 minute for chlorhexidine or 1. Wipe off povidone-iodine with alcohol and allow the alcohol to dry. For children, swab the site 3 times using only alcohol.

After this point, do not touch the skin site with any nonsterile item. Try to access the vein efficiently and collect the blood sample within 30 seconds after tourniquet placement.

Reapply the tourniquet proximal to the selected insertion site. Do not have patients make a fist or let their arm hang down during the blood sampling, because these maneuvers may cause various erroneous laboratory values eg, increased potassium, lactate, phosphate.

Apply gentle traction to the vein distally using the thumb of your nondominant hand to prevent the vein from moving. Traction may not be necessary for larger veins of the forearm or antecubital fossa. Insert the needle proximally ie, in the direction of venous blood flow , with the bevel facing up, along the midline of the vein at a shallow angle about 10 to 30 degrees to the skin.

Blood will appear in the needle hub called a blood flash when the needle tip enters the lumen of the vein. Stop advancing the needle, lower the needle to better align it with the vein, and advance it into the vein an additional 1 to 4 mm, to ensure that it stays in position during blood collection.

If no flash appears in the hub after 1 to 2 cm of insertion , withdraw the needle slowly. If the needle had initially passed completely through the vein, a flash may now appear as you withdraw the needle tip back into the lumen.

If a flash still does not appear, withdraw the needle almost to the skin surface, change direction, and try again to advance the needle into the vein. If rapid local swelling occurs , blood is extravasating. Terminate the procedure: Remove the tourniquet and the needle and apply pressure to the puncture site with a gauze pad a minute or 2 is usually adequate unless the patient has a coagulopathy.

When using vacuum tubes , push each tube fully into the tube holder, use care to avoid dislodging the needle from the vein. Fill multiple collection tubes in the proper sequence. When using a syringe , pull back on the plunger gently to avoid damaging the blood cells or collapsing the vein.

When blood collection is complete , gently hold a folded gauze square at the venipuncture site with your nondominant hand, and in one motion remove the needle and immediately apply pressure to the site with the gauze.

Remove the tourniquet if you did not do so earlier. Do not inject blood into vacuum collection tubes; allow the vacuum to draw the blood into the tube. After blood has been added to a tube, gently invert the tube 6 to 8 times to mix the contents; do not shake the tubes.

Deploy safety cover over the exposed needle. Deposit used blood-collection devices with needles into a sharps container. Do not recap non-safety needles prior to disposal unless a sharps container is not immediately available.

Use only mild tension when applying the tourniquet; it is a venous, not an arterial, tourniquet. Remember to remove the tourniquet after the blood draw. If the vein is not entered, do not try to reposition the needle by moving the tip to one side or another; this can push the vein out of the way and also damage tissue.

Instead, withdraw the needle partway before changing the angle and direction of insertion. It is no longer recommended to flex the elbow after antecubital venipuncture; this actually increases hematoma formation. Butterfly needles attached to a syringe may be preferred for difficult venipuncture eg, small veins in neonates, fragile veins in older adults.

Due to its close proximity to the brachial artery and median nerve, the basilic vein, which is located on the pinkie side of the arm, should be used only if there is not another more prominent arm vein.

If there is not an alternate vein to draw, the venipuncture should be performed distal to below the hematoma. The tourniquet should be applied between the IV site and the venipuncture site.

If drawing above the IV site is the only option, then the IV infusion must be turned off for at least 2 minutes before performing the venipuncture.

As there is still a risk that the sample could be contaminated, you must document that the specimen was drawn above proximal to an IV site and how many minutes the IV was turned off before the draw occurred. The lab may reject the specimen as contaminated based on the test results.

If you are using a tourniquet for preliminary vein selection, it should be released after one minute, left off for two minutes and then reapplied before performing the venipuncture. Procedure and Performance of a Venipuncture:  Select the proper size needle and attach it to the syringe or Vacutainer hub.

Apply the tourniquet inches above the selected puncture site. Do not place tightly or leave on for more than 1 minute. Instruct the patient to make a fist and hold it; do not have them pump their hand.

Select the venipuncture site by palpating with the gloved index finger. Do not dry the site with gauze and do not blow on the area to dry as this will contaminate the site. After cleansing the area, if it is accidentally touched before performing the venipuncture, it should be cleansed again.

Swiftly insert the needle through the skin, bevel side up, at a 15 — 30 degree angle with the skin, into the lumen of the vein. It is possible that a nerve has been punctured and possibly damaged. The venipuncture should be repeated in a different site. A Supervisor or Manager should be notified and the incident should be documented.

Forceful, direct pressure should be applied to the site for a minimum of five minutes or until the bleeding has stopped. The nursing staff should be notified, and they in turn must notify the physician.

A laboratory Supervisor or Manager should also be notified and the incident should be documented. If the blood is flowing slowly, gently adjust the angle to see if the needle is sitting up against the wall of the vein. Loosen the tourniquet, as it may be obstructing blood flow.

If you are vacutaining, try another tube — there may be no vacuum in the tube. In the case of a difficult venipuncture, an individual may make a maximum of two attempts before having someone else try.

A third stick is allowable if a partial sample has been obtained and you as the drawer feel reasonably confident that you can obtain the specimen on the next try. While the needle is still in the vein, activate the safety button with the tip of the index finger; the needle will automatically retract from the vein and the safety device will cover the needle.

If you used a needle and syringe, ask your patient or a parent to apply pressure to the site so that you can fill your tubes.

Return to your patient and assess the site of the puncture. Apply a band-aid or CoFlex to the site. Make sure the needle fully penetrates the upper-most wall of the vein; partial penetration may allow blood to leak into the tissue surrounding the vein. Adequate pressure should be applied to stop the bleeding once the phlebotomy is complete.

A hematoma can cause a post-phlebotomy compression injury to a nerve. If using a needle and syringe, avoid drawing the plunger back too forcefully. Make sure the venipuncture site is dry.

If using a blood transfer device to fill vacutainer tubes, allow the vacuum to pull the blood into the tubes; do not use the plunger on the syringe to force the blood into the tubes more quickly. Excessive probing uncalculated side-to-side movement with the needle should be avoided.

Have appropriate back up staff as available. If there is any concern regarding injury, contact nursing for Inpatients and follow the Policy for Proper Handling of an Uncooperative Patient.
